IF INCOMPLETE, YOUR REGISTRATION WILL NOT BE ACCEPTED

YOU MUST HAVE A REC CARD FROM THE CITY OF CLEARWATER TO PLAY ON A CITY OF CLEARWATER FIELD.
IMPORTANT INFORMATION (NO REFUNDS): Refunds will not be given for any reason other than your child not being placed on a team due to space limitations. This includes that there will NOT be refunds for any reason, including but not limited to illness, relocation, injury, child decides not to play, parent decides not to let the child play and/or dismissal from CJT due to rule or code of conduct violations.
